1. How many beds are in the facility?
We have 134 beds, all of which are certified for skilled nursing care.

2. Do you offer private rooms?
Yes, we offer private and semiprivate rooms.

3. Does a resident have to have a physician?
Residents are required to have a local physician and the physician must be willing to visit the resident in the facility every 60 days. The facility will provide assistance in locating a local physician if needed.

4. What forms of payment does the facility accept?
The facility accepts Medicaid, Medicare, and private pay.

5. How does the facility staff?
The facility staffs with the quantity of employees that it takes to give quality resident care. The facility has approximately 134 employees with about 100 of those being direct care staff.

6. What is your policy regarding visiting after hours?
Scheduled visiting hours are 10:00 AM to 8:00 PM, but we try to be flexible in this area as this is the resident’s home. Family members can visit any time as long as visitation in a resident’s room does not interfere with the roommate’s schedule or privacy.

7. Do you offer exercise classes?
Yes, we have an exercise class every morning and every weekday afternoon. In addition, our restorative aides perform one-on-one range of motion, strengthening, and ambulation workouts throughout the day.

8. Do you provide transportation?
Transportation is available for medical appointments. Family members are encouraged to meet us at the doctor’s office when possible.

9. Can residents have their own phone?
Yes, residents may have their own phone in their room. The family simply has to call the phone company and make arrangements for the connection.

10. Do you provide counseling for residents?
We have a full time Social Worker who provides ongoing emotional support and crisis intervention. Outside consultants are available to provide individual psychotherapy, as well as psychiatric evaluations and treatment.

11. Are the residents allowed to go home for visits?
Residents are able to go on community outings and go home for visits. The amount of time a resident can be out of the facility depends on the type of payment plan they are on. Residents who receive Medicaid can be out of the facility for up to 72 hours. Residents who are on Medicare cannot stay out overnight and any outings must be therapeutic in nature. Residents who are paying privately can stay gone as long as they choose.

12. Do you accept residents that smoke?
We are a non-smoking facility. We do not keep smoking materials or assist with smoking. A resident may smoke outside with visitors, but by regulation may not keep smoking materials.

13. What type of activities do you offer?
Some of the activities provided are religious services, games, musical programs, birthday parties, theme dinners, social gatherings, as well as holiday parties and events. Families are included in annual covered dish luncheons held at Easter and Thanksgiving. There is an activity calendar posted at the nurse’s station and individual calendars are available each month.

14. Do you have a beauty shop?
Yes, and our beautician is in the facility on Tuesdays and Wednesdays. Services are provided for a fee. Family members may use the beauty shop as well by prior arrangement with the Activity Department.

15. Do you have pets?
Yes, we have a dog, birds, and cats that reside in the facility. Visiting pets are allowed.

16. Do you provide laundry service?
Yes, laundry is included in the daily rate. The laundry runs daily so clothing is generally washed and returned in the same day. The family is welcome to do the resident’s laundry if preferred.

17. Do you provide Hospice care?
We have contracts with several Hospice providers in the area so your loved one is provided with end of life choices and support.
